Student Forms

The following forms are required to be completed by the parent/guardian of students attending the MCVSD Career Center.

  1. Student Emergency Card/Information – please provide us with a parent/guardian email address

  2. Academic Commitment

  3. Acceptable Use Policy

  4. Family Educational Rights and Privacy Act (FERPA)

  5. Publicity Release Statement

  6. Safety Pledge

  7. Student Handbook 

    1. Student Handbook Signature Page

  8. Medical Summary Form (confidential)

The above list of forms can be dowloaded and completed OR completed in PowerSchool. 

All returning students have been issued their PowerSchool credentials while new students will be issued their PowerSchool credentials

In order to complete the forms, login into PowerSchool. Select FORMS from the Navigation list on the left-hand side. All required forms will be displayed for you to revise or edit. Please be sure to click on SUBMIT at the bottom-right of each completed form to save your changes.

It is critical that you complete and return the required forms to the Career Center by September 18, 2024. Failure to do so will result in your child being excluded from full participation in their vocational program.
